Your physicians medical opinion is; you are suffering with a terminal medical condition from which you are expected to die expected to die within six months, even with available life- sustaining treatment provided in accordance with prevailing medical care standards; or

In your physicians medical opinion, you have a condition that is irreversible and you cannot make decisions or care for yourself and are expected to die without life-sustaining treatment provided in accordance with prevailing medical care standards.

Often people ask "Do I need a Directive to Physicians?" The answer to this question depends entirely on the amount of medical treatment they wish to receive if a series injury or illness where to occur.

When deciding whether to execute a Directive, the person may consider what burdens or hardships of treatment they will be willing to endure for a certain health benefit.
